Waiver and release: We, the undersigned, for our selves, our heirs, our executors, and administrators, waiver and release and forever discharge Brighton High School and its camp staff, officers, agents, representatives, employees, successors, and assigns of and from any and all rights and claims for damages to personal property which may be sustained or occur during participation in camp activities, or from camp, whether paid damages, injury, or loss due to negligence or not. I certify that the applicant is in good physical condition to take part in the youth camp. I hereby give my permission for my son to participate in the described camp. I have adequate insurance coverage in the event of accident or injury. I also give the camp staff to use images of my child in promotional activities where the name of my child is not used within a picture.
